ACFI Specialist / Care Funding Business Partner

With 44 aged care homes throughout NSW, QLD, Victoria and SA we are proud to introduce a really fresh approach in residential aged care and are excited to bring a unique design and modern approach to aged care throughout Australia.Overseeing a number of our Homes, predominately in NSW plus 3 in QLD, the role is mainly based in Sydney with some irregular regional NSW & QLD travel. We are seeking a Registered Nurse with excellence in gerontology to work in our Clinical Assessment Team. The Care Funding Business Partner (ACFI Specialist) is responsible for mentoring, implementing and monitoring the Allity framework of assessment, care planning and ACFI systems which optimise care excellence for residents and care funding outcomes.The role is responsible for the collation of assessment and documentation to reflect individualized care needs. We are looking for someone with the following:

Current Nursing registration with AHPRA

Significant aged care experience

Expertise in clinical assessment of elderly residents

Computer literacy and excellent documentation skills

Experience working with Medicare online systems

Ability to contribute in a leadership role and team environment

Ability to mentor, train and develop the assessment of skills of care personnel

Knowledge of relevant Aged Care legislation and best practice guidelines

Demonstrated knowledge of ACFI business rules

Experience with validation process for Aged Care Funding Instrument

Experience with electronic care documentation systems

Good communication skills – written and verbal

Excellent organisational and time management skills

Proven experience with quality and in delivering continuous improvement projects

Must be flexible to travel to various Allity Homes in NSW & occasionally interstate.

Own car is required and KMs are reimbursed.
